Abstract

Diabetes remains one of the most challenging chronic illnesses, with many patients struggling to consistently manage their condition, access reliable health data, or maintain communication with healthcare providers. To address these issues, DiabeFit introduces a centralized solution that supports both patients and doctors in monitoring health status and improving care delivery. DiabeFit is dedicated to supporting individuals with diabetes through a blend of innovative technology and personalized care. Our project offers a user-friendly platform that helps monitor blood glucose levels, track medication, and provide customized nutrition and exercise plans. Our methodology integrates AI algorithms capable of analyzing patient data to predict the risk of diabetes, alongside a chatbot assistant that delivers real-time responses and personalized information. The application also provides access to educational resources, dietary recommendations, and tracking tools to enhance user engagement and self management. In conclusion, DiabeFit simplifies chronic disease management, encourages preventive care, and bridges the communication gap between patients and physicians. Future enhancements will focus on expanding the AI's predictive capabilities, adding more interactive tools, and integrating with wearable health devices to deliver even more comprehensive care.
